Peripheral Blood of Patients with HIV-Infection and Oral Disease

Objectives: We conducted a study to estimate the frequency of hemocytopenia among HIV-positive patients with oral pathology.

Methods: The research was made among 44 HIV-positive patients, who were diagnosed different oral diseases. Their peripheral blood was analyzed, and the quantity of leucocytes (WBC), neutrophils (NEUT), haemoglobin (HGB), platelets (PLT), CD4 and CD8 lymphocytes were counted.

Results: The group included 31 men (70.4%). The mean age was 27.5±1.0 years old. 37 patients (84.1%) had hepatitis virus infection. The mean duration of HIV-infection was 3.3±0.3 years. 14 men (31.8%) received HAART; 4 patients (9.1%) had an AIDS diagnosis. In 25 cases the CD4 level was 0,200-0,500x109 /l, according to the previous test. 28 (63.6%) cases had gingivitis; 9 (18.2%) cases had periodontitis, including necrotizing – 6 (13.6%) cases; candidiasis – 4 (9,1%) cases; herpetic stomatitis – 4 (9.1%) cases. Laboratory analysis demonstrated: thrombocytopenia (<100x109/l) in 4 (9.1%) cases; neutropenia (<1,0x109/l) – 1 (2.3%) case. The mean value for WBC was 6.044x109/l, for PLT – 180x109/l; for NEUT – 3.867x109/l; for HGB – 147.2 g/l, mean rate for CD4 was 0.413 x109 /l. 2 patients (4,5%) with a duration of HIV-infection more than 6 years, who didn’t receive HAART got mixed oral infections (like gingivitis, candidiasis and herpetic stomatitis).

Conclusion: According to our results the serious haemocytopenia (excluding CD4) was registered among insignificant number of HIV-positive patients with oral disease. Probably there is the possible connection of oral disease (including mix) with the duration of HIV-infection and absence of HAART.
